What is Medi-Cal share of cost?

It is the amount of covered or qualifying medical costs you must pay or owe before Medi-Cal pays for covered care that month. It is often called SOC. It is not a monthly insurance premium.

Why do I have a share of cost?

You have a share of cost when Medi-Cal counts more monthly income than the maintenance need level for your household, and you do not qualify for a program without one. It is common for people with Social Security or pension income just above the free Medi-Cal limits.

How does share of cost work?

Each month, Medi-Cal subtracts a maintenance need level from your countable income. What is left is your share of cost. Once your medical bills for the month reach that amount, Medi-Cal pays for covered care for the rest of the month.

Is share of cost the same as spend down?

Yes. "Spend down" is the general Medicaid term. California calls its Medically Needy spend down a "share of cost." See the comparison.

Is there a Medi-Cal share of cost calculator?

Can dental insurance lower my share of cost?

An eligible premium may lower the income Medi-Cal counts under some program rules. Ask your county whether a premium deduction could move you into a program with no share of cost. Do this before buying a plan.

Do I need insurance that costs as much as my share of cost?

Not necessarily. The key number may be how far your countable income is over the limit for a no-share-of-cost program. That can be much smaller than the SOC on your letter. Ask the county for both numbers.

Does buying insurance guarantee a $0 share of cost?

No. The county must check your program, household, income, deductions, and other eligibility rules. A new policy alone does not change your case.

Is this an official Medi-Cal website?

No. Share of Cost Advisors is an independent educational website. County offices make benefit decisions. This website does not enroll you in Medi-Cal or insurance.

Can I send my paperwork here?

Do I still need to apply for IHSS?

Yes. Buying insurance is not an IHSS application. IHSS has its own application, health certification, and county assessment.

Is private dental insurance the same as Medi-Cal Dental?

No. A private dental policy is separate insurance. Medi-Cal Dental is a state benefit. Check your existing coverage before choosing another policy.

Will a new policy fix earlier months?

Do not assume so. A policy has its own coverage start date. Ask the county which months can be reviewed and what proof it needs. Buying coverage now does not automatically erase an earlier share of cost.

What if the county’s decision looks wrong?

Ask for the calculation in writing. Check the appeal instructions and deadline on your Notice of Action. Seek help promptly from a benefits counselor or legal aid organization; do not let a deadline pass while waiting for an informal reply.
