IHSS share of cost: how it affects caregiver pay

Know how a Medi-Cal cost can affect the person who helps you at home.

Short answer

If an IHSS recipient has an unmet Medi-Cal share of cost, the state can deduct that amount from the IHSS provider’s paycheck, and the recipient owes it to the provider directly. Lowering the Medi-Cal share of cost lowers what the recipient owes the caregiver.

How does share of cost work with IHSS?

IHSS is paid through Medi-Cal. If you have a share of cost, part of your IHSS provider’s pay may be your responsibility. The state can subtract the remaining share of cost from the provider’s payment. Read the share-of-cost notice and timesheet information together.

A simple example

$300 left for the month

Suppose your share of cost is $500. Qualifying medical costs of $200 have already been credited before the IHSS timesheet is processed. The remaining $300 may be deducted from the provider’s state payment. You would owe that amount to the provider.

Talk before payday

Keep your provider informed. Save your medical receipts and share-of-cost notices. If the amount looks wrong, call the county IHSS office and ask how it was calculated.

A new policy does not approve IHSS

IHSS requires its own county review. Coverage, authorized hours, and caregiver enrollment are separate matters. A lower share of cost does not mean every hour of care will be paid.
